spa:Objetivo: Entre los patógenos gramnegativos, Pseudomonas aeruginosa
resistente es la causa más común de infecciones nosocomiales y de asistencia
sanitaria, representando un problema de salud pública a nivel mundial por lo que
el objetivo de esta investigación es determinar cuáles son los factores de riesgo
relacionados con el aislamiento de Pseudomonas aeruginosa resistente, en un
hospital de tercer nivel de Caldas, durante el año 2020. Metodología: Se trata
de un estudio observacional, retrospectivo, transversal, analítico. en donde se
tomaron todos los aislamientos de Pseudomonas aeruginosa registrados en el
censo epidemiológico del equipo de investigación de infecciones del S.E.S.
Hospital Universitario de Caldas, de pacientes que ingresaron durante el año
2020, independiente del servicio, se depuraron y se analizaron los datos. La base
de datos fue realizada en el programa Excel (Microsoft Corporation), y el análisis
estadístico se realizó mediante el programa estadístico IBM SPSS 22 (IBM
Corp.). Para la descripción epidemiológica y clínica de la muestra estudiada se
utilizó la distribución de frecuencias y se realizó un análisis bivariado de las
variables de interés. Para determinar los factores que se asocian con la
probabilidad de adquirir infección por Pseudomonas aeruginosa con algún grado
de resistencia, se evaluó la relación entre los posibles factores de riesgo
mediante regresión logística. Los resultados son expresados como razón de
disparidad OR con sus respectivos intervalos de confianza (IC) del 95%.
Resultados: El grupo decenal con mayor número de casos, correspondió al de
la edad comprendida entre los 70 y los 79 años. La mayoría de los pacientes
procedía de la ciudad de Manizales (76.7%). El régimen de aseguramiento
predominante fue el contributivo. La comorbilidad con el mayor número de casos
fue la hipertensión arterial (63.3%), seguida por la EPOC (Enfermedad Pulmonar
Obstructiva Crónica) con un 18.3% y la ERC (Enfermedad Renal Crónica) con
un 11.7%. El 14.3% de los pacientes tenía un elemento invasivo (sondas, tubos
endotraqueales, otros). El servicio que más realizó aislamientos de
Pseudomonas Aeruginosa fue la Unidad de Cuidados Intensivos, donde se
realizó el 45% de los mismos. La muestra de la cual se obtuvo la mayoría de los
aislamientos fue la de orina (26.7%). Respecto al perfil de resistencia antibiótica,
la mayor proporción de sensibilidad se presentó frente a la colistina, 96.7%, y la
menor, frente al aztreonam; 26.7%. Se presentaron 14 decesos de pacientes
durante la estancia hospitalaria. Se encontró significancia estadística para la
variable de la edad media (p de 0.02) y el aislamiento de Pseudomonas
aeruginosa resistente; en la regresión logística no se encontraron variables
adicionales estadísticamente significativas. El sexo masculino, diabetes mellitus,
régimen subsidiado y la variable edad, se relacionaron estadísticamente con
Pseudomonas aeruginosa con algún grado de resistencia antibiótica.

aeruginosa is the most common cause of nosocomial and health care infections,
representing a worldwide public health problem, so the objective of this research
is to determine the risk factors related to the isolation of resistant Pseudomonas
aeruginosa, in a third level hospital in Caldas, during the year 2020.
Methodology: This is an observational, retrospective, cross-sectional, analytical
study where all the isolates of Pseudomonas aeruginosa registered in the
epidemiological census of the infection investigation team of the S.E.S. University
Hospital of Caldas, of patients who were admitted during the year 2020,
regardless of the service, the data were refined and analyzed. The database was
created using the Excel program (Microsoft Corporation), and the statistical
analysis was performed using the statistical program IBM SPSS 22 (IBM Corp.).
For the epidemiological and clinical description of the studied sample, the
frequency distribution was used and a bivariate analysis of the variables of
interest was performed. To determine the factors associated with the probability
of acquiring P. aeruginosa infection with some degree of resistance, the
relationship between possible risk factors was evaluated using logistic
regression. The results are expressed as odds ratio OR with their respective
confidence intervals (CI) of 95%. Results: The ten-year group with the highest
number of cases corresponded to the age group between 70 and 79 years. Most
of the patients came from the city of Manizales (76.7%). The predominant
insurance regime was contributory. The comorbidity with the highest number of
cases was arterial hypertension (63.3%), followed by COPD with 18.3% and CKD
with 11.7%. 14.3% of the patients had an invasive element (probes, endotracheal
tubes, others). The service that performed the most isolations of Pseudomonas
Aeruginosa was the Intensive Care Unit, where 45% of them were performed.
The sample from which most isolates were obtained was urine (26.7%).
Regarding the antibiotic resistance profile, the highest proportion of sensitivity
was presented against colistin, 96.7%, and the lowest, against aztreonam;
26.7%. There were 14 patient deaths during the hospital stay. Statistical
significance was found for the variable of mean age (p of 0.02) and the isolation
of resistant Pseudomonas aeruginosa; no statistically significant additional
variables were found in logistic regression. Male gender, diabetes mellitus,
subsidized regimen and the age variable were statistically related to
Pseudomonas aeruginosa with some degree of antibiotic resistance.
Conclusions: In a third level hospital in Caldas, it was determined that risk
factors such as service, origin, invasive procedures and the biological sample
where the isolation was performed and comorbidities other than diabetes
mellitus, have no statistically significant relationship with the fact of obtain an
isolate of Pseudomonas aeruginosa resistant or with some degree of resistance.
