dc.description.abstract | spa:La Puntuación Nacional de Alerta Temprana (NEWS/NEWS2) se ha adoptado en todo el Servicio Nacional de Salud (NHS) del Reino Unido como método para evaluar a los pacientes en riesgo de deterioro clínico. Clasificar adecuadamente el riesgo de los pacientes ingresados en el servicio de urgencias, que requieren traslado a la Unidad de Cuidados Intensivos, intubación orotraqueal y muerte, es fundamental para mejorar la calidad de la atención y los resultados clínicos. Se evaluó la validez predictiva de la escala NEWS-2 en una población de pacientes adultos que ingresaron en un servicio de urgencias nivel III de la ciudad de Manizales. Utilizando un punto de corte de 5 puntos, NEWS-2 tuvo una Sensibilidad: 63%, Especificidad: 81%, VPP: 64%, VPN: 80%, LR+: 3,32, LR-: 0,46. Ajustando el punto de corte a 4 puntos, según las coordenadas de nuestras curvas ROC, obtuvimos una Sensibilidad: 79%, Especificidad: 75%, VPP: 63%, VPN: 87%, LR+: 3,16, LR-: 0 , 28 y un AUC de 0,80. El NEWS-2 discrimina razonablemente el riesgo de que los pacientes ingresados en urgencias requieran traslado a UCI, intubación orotraqueal o muerte en las próximas 24 horas. NEWS-2 tiene el potencial de convertirse en una herramienta de detección sistemática para todos los pacientes ingresados en el servicio de urgencias de nuestra región, mejorando la seguridad de la atención. | spa |
dc.description.abstract | eng:The National Early Warning Score (NEWS/NEWS2) has been adopted across the UK National Health Service (NHS) as a method of assessing patients at risk of clinical deterioration. Properly classifying the risk of patients admitted to the emergency department, requiring transfer to the Intensive Care Unit, orotracheal intubation, and death, is essential to improve the quality of care and clinical outcomes. We evaluated the predictive validity of the NEWS-2 scale in a population of adult patients who were admitted to a level III emergency department in the city of Manizales. Using a cut-off point of 5 points, NEWS-2 had a Sensitivity: 63%, Specificity: 81%, PPV: 64%, NPV: 80%, LR+: 3.32, LR-: 0.46. Adjusting the cut-off point to 4 points, according to the coordinates of our ROC curves, we obtained a Sensitivity: 79%, Specificity: 75%, PPV: 63%, NPV: 87%, LR+: 3.16, LR-: 0, 28 and an AUC of 0.80. The NEWS-2 reasonably discriminates the risk of patients admitted to the emergency room requiring transfer to the ICU, orotracheal intubation, or death in the next 24 hours.
