dc.description.abstractspa: La capacidad funcional es un parámetro influido y soportado por múltiples sistemas, donde confluyen variables biológicas, sociales y ambientales (1). Resulta de especial interés intentar determinar que variables internas y externas pueden influir en la capacidad funcional, máxime si incluye condiciones potencialmente modificables, sabiendo que estas pueden ser determinantes en el curso de discapacidad de los adultos mayores (2) Se ha descrito la relación de desenlaces adversos en funcionalidad y los factores de riesgo cardiovascular (FRCV) como la diabetes, hipertensión, dislipidemia, pero no se ha determinado que papel puede jugar la confluencia de estas condiciones en la capacidad funcional de un mismo individuo. En el presente estudio se plantea como objetivo explorar la potencial correlación entre la carga de FRCV evaluada por medio de la FRS (Framingham general cardiovascular disease risk score) en adultos mayores y la capacidad funcional cuantificada por medio del SPPB (Short Physical Performance Battery), así como los parámetros clínicos y de laboratorio que puedan explicar vías de asociación. El IMIAS fue un estudio prospectivo que inicio en el año 2012. Tuvo como población objeto a personas mayores que vivían en la comunidad, fue realizado en 5 ciudades de diferentes partes del mundo. Se llevo a cabo aplicando un instrumento de recolección multidimensional incluyendo datos de factores de riesgo y eventos cardiovasculares, se tomaron muestras para análisis sanguíneos de laboratorio con diferentes parámetros de interés e información relacionada que permitieron calcular el FRS. Dentro de las pruebas clínicas se aplicó el SPPB con posteriores seguimientos de la evolución en los años 2014 y 2016. Se realizó un análisis estadístico de carácter exploratorio tratando de correlacionar la carga de FRCV categorizado en tres grupos de riesgo por medio de la FRS y la capacidad funcional cuantificada mediante el puntaje total del SPPB en cada uno de los años de seguimiento entre los grupos, para intentar determinar la existencia de diferencias en la capacidad funcional entre los grupos en cada uno de los años de seguimiento. También se analizó la diferencia entre los grupos de riesgo por cada uno de los tres componentes del SPPB. Se lograron determinar diferencias estadísticamente significativas, con una peor capacidad física entre los grupos de más alto riesgo, que al parecer se acentúan en el tiempo, las pruebas del SPPB que al parecer explican en mayor medida estas diferencias, la velocidad de la marcha e incorporarse de una silla en 5 repeticiones. El grupo de riesgo cardiovascular alto mostro puntajes más bajos en la prueba de rendimiento cognitivo. La edad no explica las diferencias encontradas. Se necesitan estudios adicionales que logren determinar si en esta cohorte el riesgo cardiovascular explica un mayor deterioro de la capacidad funcional en el tiempo y si esta no es influenciada por factores como el deterioro cognitivo.spa
dc.description.abstracteng: Functional capacity is a parameter influenced and supported by multiple systems, where biological, social and environmental variables converge (1). It is of special interest to try to determine which internal and external variables can influence functional capacity, especially if it includes potentially modifiable conditions, knowing that these can be determinants in the course of disability in older adults (2) The relationship of outcomes has been described adverse effects on functionality and cardiovascular risk factors (CVRF) such as diabetes, hypertension, dyslipidemia, but it has not been determined what role the confluence of these conditions can play in the functional capacity of the same individual. The objective of this study is to explore the potential correlation between the burden of CVRF evaluated by means of the FRS (Framingham general cardiovascular disease risk score) in older adults and the functional capacity quantified by means of the SPPB (Short Physical Performance Battery), as well as clinical and laboratory parameters that may explain pathways of association. The IMIAS was a prospective study that began in 2012. Its target population was elderly people living in the community, it was carried out in 5 cities in different parts of the world. It was carried out by applying a multidimensional collection instrument including data on risk factors and cardiovascular events, samples were taken for laboratory blood tests with different parameters of interest and related information that allowed calculating the FRS. Within the clinical tests, the SPPB was applied with subsequent follow-up of the evolution in the years 2014 and 2016. An exploratory statistical analysis was carried out, trying to correlate the burden of CVRF categorized into three risk groups through the FRS and the functional capacity quantified by means of the total score of the SPPB in each of the years of follow-up between the groups, to try to determine the existence of differences in functional capacity between the groups in each of the years of follow-up. The difference between the risk groups for each of the three SPPB components was also analyzed. It was possible to determine statistically significant differences, with a worse physical capacity between the highest risk groups, which seem to be accentuated over time, the SPPB tests that seem to explain these differences to a greater extent, the speed of walking and getting up of a chair in 5 repetitions. The high cardiovascular risk group showed lower scores on the cognitive performance test. Age does not explain the differences found. Additional studies are needed to determine whether in this cohort cardiovascular risk explains a greater deterioration in functional capacity over time and whether it is not influenced by factors such as cognitive impairment.eng
