Sentido y significado del cuidado de pacientes adultos mayores con delirium, para auxiliares de enfermería del Hospital Pablo Tobón Uribe

spa:Delirium: síndrome neuropsiquiátrico complejo, agudo, fluctuante, multifactorial y transitorio, caracterizado por alteración de conciencia y desorden de pensamiento. Se presenta en ancianos y ambientes hospitalarios, por lo que sus cuidadores directos, es decir el Personal Auxiliar de Enfermería (AE), debe reconocerlo, asumirlo y aportar a su resolución y buen desarrollo. Por lo anterior, es válido preguntarse; ¿qué significa para AE cuidar pacientes con delirium y cuál es el sentido que le dan durante la hospitalización?
El objetivo de este estudio fue comprender el significado y sentido que tiene para el Personal Auxiliar de Enfermería, desde su experiencia y saberes adquiridos, el cuidado de pacientes mayores de 60 años hospitalizados con delirium.
Este es un estudio cualitativo, comprensivo, con enfoque interpretativo hermenéutico, en el que participaron 6 auxiliares de enfermería y en el que se realizaron entrevistas individuales semiestructuradas a profundidad. El análisis de los datos se realizó en tres momentos: descriptivo (transcripción), analítico (codificación) e interpretativo (integración de categorías).
El sentido y significado que tiene para AE cuidar a pacientes mayores de 60 años hospitalizados con delirium se compuso a partir de: la experiencia de cuidar, que incluye tiempo laboral, número de pacientes atendidos y conocimientos adquiridos; el enigma del delirium, lo cual hizo evidente la incertidumbre generada por falta de formación especializada y el tiempo que debe invertirse para cuidarlo y humanizarlo; la construcción de conocimiento, la cual incluyó formación y acompañamiento del Hospital y transmisión de conocimientos; la satisfacción laboral, que conjugó afinidad, compromiso y consciencia en la atención de pacientes, y reflejó seguridad y calidad del cuidado; las personas mayores cuidadas, en quienes se comparó dependencia y carga de cuidado con apoyo de redes sociales y descarga del cuidado para enfermería; y de cara al delirium, estrategias de afrontamiento construidas desde una formación empírica y propuestas para una estructuración.
Como conclusión, AE significa y da sentido al cuidado desde su experiencia personal y la construcción constante de conocimiento en su vida laboral.
Esta investigación pretende aproximarse a la visión del cuidado que tiene el AE, lo cual ha sido poco estudiado hasta este momento. eng:Delirium: complex, acute, fluctuating, multifactorial and transitory neuropsychiatric syndrome, characterized by altered consciousness and thought disorder. It occurs in the elderly and in hospital environments, so its direct caregivers, i.e. the Auxiliary Nursing Staff (AN), must recognize it, assume it and contribute to its resolution and good development. Therefore, it is valid to ask: what does it mean for the auxiliary nursing staff to care for patients with delirium and what is the meaning they give to it during hospitalization?
The objective of this study was to understand the meaning and significance it has for the auxiliary nursing staff, from their experience and acquired knowledge, the caring of patients over 60 years of age hospitalized with delirium.
This is a qualitative, comprehensive study, with a hermeneutic interpretative approach, in which 6 auxiliary nurses participated and in which individual semi-structured in-depth interviews were conducted. The data analysis was carried out in three stages: descriptive (transcription), analytical (coding) and interpretative (integration of categories).
The meaning and significance that has for the AN the caring of patients over 60 years of age hospitalized with delirium was composed from: the experience of caring, which includes work time, number of patients cared for and knowledge acquired; the enigma of delirium, which made evident the uncertainty generated by the lack of specialized training, the amount of time invested in its care and the need to humanize it; the construction of knowledge, which included training and accompaniment by the Hospital and transmission of knowledge; job satisfaction, which combined affinity, commitment and conscientiousness in patient care, and reflected safety and quality of care; the elderly cared for, in whom dependence and burden of care were compared with support from social networks and care discharge for nursing; and in the face of delirium, coping strategies constructed from empirical training and proposals for structuring.
As a conclusion, AN gives meaning to caring from their personal experience and the constant construction of knowledge in their working life.
This research aims to approach the vision of care that the AN has, which has been little studied so far.
