dc.description.abstractspa:Antecedentes: La anestesia regional es una técnica usada para eliminar o disminuir el empleo único de sedación o uso de anestesia general, reduciendo los riesgos asociados especialmente en pacientes con alta carga mórbida, incluyendo a los ancianos. En la actualidad existe un déficit en literatura o soporte científico respecto al desarrollo de tecnologías de asistencia en anestesia regional o inteligencia artificial para el uso clínico diario, en consecuencia, el presente proyecto pretende determinar la validez de criterio de una herramienta tecnológica desarrollada para la identificación automática del nervio ciático por enfoque poplíteo a partir de imágenes ultrasonográficas. Dicho instrumento tecnológico es producto del macroproyecto para la asistencia tecnológica en anestesia regional desarrollado por los grupos de medicina hospitalaria de la institución S.E.S Hospital Universitario de Caldas, grupo de Automática de la universidad Tecnológica de Pereira y el grupo de Control y Procesamiento digital de la Universidad Nacional de Colombia. Metodología: Se incluyeron pacientes adultos programados para cirugía electiva y urgente, no inestables, en la institución S.E.S Hospital Universitario de Caldas. A dichos pacientes seleccionados se les realizó un vídeo del estudio ultrasonográfico del nervio ciático por enfoque poplíteo, el cual luego fue procesado en imágenes. Dichas imágenes fueron etiquetadas por un experto en anestesia regional (estándar de referencia) y por el software desarrollado, donde dichas etiquetas finalmente se compararon a través de métricas como la sensibilidad, especificidad, coeficiente de Dice (CD) e intersección sobre unión (IoU). Resultados: Se incluyeron 24 pacientes en el estudio, 12 hombres y 12 mujeres, de los cuales se recogieron 223 imágenes para ser evaluadas por el estándar de oro y el software desarrollado. Se alcanzó una sensibilidad de 0.96 (IC95% 0.93-0.99) y una especificidad de 0.99 (IC95% 0.99-0.99). Se identificó además una correlación positiva entre el índice de DICE y el índice de intersección sobre la unión del software y el peso e IMC; por el contrario, una correlación negativa entre dichas métricas y la edad. Conclusión: El software desarrollado tiene una alta sensibilidad y especificidad para la identificación del nervio ciático evaluado por enfoque poplíteo; además posee una buena capacidad para identificar de forma precisa la mayor parte de las estructuras nerviosas evaluadas.spa
dc.description.abstracteng:Background: Regional anesthesia is a technique used to eliminate or decrease the single use of sedation or use of general anesthesia, reducing the associated risks especially in patients with high morbid burden, including the elderly. There is currently a deficit in literature or scientific support regarding the development of assistive technologies in regional anesthesia or artificial intelligence for daily clinical use, consequently, the present project aims to determine the criterion validity of a technological tool developed for the automatic identification of the sciatic nerve by popliteal approach from ultrasonographic images. This technological tool is a product of the macroproject for technological assistance in regional anesthesia developed by the groups of hospital medicine of the institution S.E.S Hospital Universitario de Caldas, the group of Automatics of the Technological University of Pereira and the group of Control and Digital Processing of the National University of Colombia. Methodology: Adult patients scheduled for elective and urgent surgery, not unstable, in the institution S.E.S Hospital Universitario de Caldas were included. These selected patients underwent a video of the ultrasonographic study of the sciatic nerve by popliteal approach, which was then processed into images. These images were labeled by an expert in regional anesthesia (reference standard) and by the developed software, where these labels were finally compared through metrics such as sensitivity, specificity, Dice coefficient (CD) and intersection over union (IoU). Results: 24 patients were included in the study, 12 males and 12 females, from which 223 images were collected to be evaluated by the gold standard and the developed software. A sensitivity of 0.96 (95%CI 0.93-0.99) and a specificity of 0.99 (95%CI 0.99-0.99) were achieved. A positive correlation was also identified between the DICE index and the intersection over union index of the software and weight and BMI; conversely, a negative correlation was identified between these metrics and age. Conclusion: The developed software has a high sensitivity and specificity for the identification of the sciatic nerve evaluated by popliteal approach; it also has a good capacity to accurately identify most of the evaluated nerve structures.eng
