dc.description.abstract | spa: Objetivo: Describir el rendimiento técnico y clínico de una aplicación de soporte de decisiones para anestesia total intravenosa (TIVA) con bombas de infusión convencionales. Métodos: Se realizó un estudio observacional, prospectivo y descriptivo, consistente en una serie de casos de pacientes entre 18 y 65 años llevados a cirugía bajo TIVA mediante “asa asistida” (así nombramos a nuestra técnica de TIVA con bombas de infusión convencionales y el uso de nuestra aplicación de soporte de decisiones clínicas) en un hospital en la ciudad de Manizales. Se incluyeron pacientes ASA I y II, programados para procedimientos no cardiacos de riesgo bajo o intermedio. Se evaluó el desempeño clínico del asa asistida, basado en monitoria de densidad espectral (SEDLine Masimo®) y la proporción de pacientes a los cuales se debió modificar la perfusión de medicamentos, diferente al instructivo dado por nuestra aplicación. Resultados: Se recolectaron datos de 40 pacientes, 32 fueron de sexo femenino (80%) con edad promedio de 47.7 años. El promedio de tiempo por fuera de rango de PSI entre 20 y 50 fue de 2.65 minutos (DE 0.78, IC95% 1.06-4.23), correspondientes a 3.78% del total de tiempo anestésico. En ningún paciente hubo necesidad de modificar la técnica anestésica. El promedio del porcentaje de decisiones que siguió el anestesiólogo del total de las que la aplicación le generó en todos los pacientes fue del 93.9% (DE 1.2, IC95% 91.3 - 96.4). Conclusiones: El rendimiento técnico y clínico de nuestra nueva aplicación para ejecutar una nueva manera de suministrar anestesia total intravenosa en “asa asistida” es satisfactorio, preciso y seguro en pacientes adultos que son llevados a cirugías no cardíacas de bajo y mediano riesgo. | spa |
dc.description.abstract | eng: Objective: Describe the technical and clinical performance of a decision support application for total intravenous anesthesia (TIVA) with conventional infusion pumps. Methods: An observational, prospective and descriptive study was carried, consisting of a series of cases of patients between 18- and 65-years old undergoing surgery under TIVA using "assisted loop" (this is how we named our TIVA technique with conventional infusion pumps and the use of our clinical decision support application) in a hospital in the city of Manizales. ASA I and II patients scheduled for low or intermediate risk non-cardiac procedures were included. The clinical performance of the assisted loop was evaluated, based on spectral density monitoring (SEDLine Masimo®) and the proportion of patients whose drug infusion had to be modified, different from the instructions given by our application. Results: Data from 40 patients were collected, 32 were female (80%) with an average age of 47.7 years. The average time out of the PSI range between 20 and 50 was 2.65 minutes (SD 0.78, 95% CI 1.06-4.23), corresponding to 3.78% of the total anesthetic time. There was no need to modify the anesthetic technique in any patient. The average percentage of decisions followed by the anesthesiologist out of the total number generated by the application in all patients was 93.9% (SD 1.2, 95% CI 91.3 - 96.4).
