dc.description.abstract | spa:El presente estudio tuvo como propósito definir las características del tratamiento trombolítico en el área de urgencias de un centro de atención de tercer nivel de la ciudad de Manizales durante los últimos cinco años, para lo cual se evaluaron las características sociodemográficas y clínicas de la población, así como las complicaciones del tratamiento trombolítico, y finalmente, la efectividad de la trombolisis en el área de urgencias para concluir con el análisis de la relación entre las características sociodemográficas, las complicaciones y la efectividad de la trombolisis como tratamiento, en el área de urgencias de un centro de atención de tercer nivel de la ciudad de Manizales entre el 2014 y 2019. Al preguntarnos por ¿Cuáles son las características del tratamiento trombolítico, en el área de urgencias de un centro de atención de tercer nivel de la ciudad de Manizales durante el 2014 y 2019?, nos encontramos con la necesidad de abordar esta temática, desde una perspectiva metodológica correlacional, ya que el estudio de los casos de atención clínica intrahospitalaria del centro de atención de tercer nivel en el cual se realizó la investigación se hizo mediante el análisis de información contenida en la historia clínica de los pacientes. Se analizaron las condiciones sociodemográficas y clínicas de la población, así como las complicaciones y la efectividad de la trombolisis en el área de urgencias del centro de atención de tercer nivel de la ciudad de Manizales escogido para el estudio. Como parte del estudio, se pudo identificar que ambos sexos, presentan una media similar que se acerca a los 60 años. En este caso, las mujeres se caracterizan por una distribución mucho más amplia con relación a los hombres, por lo que se puede decir que presentan eventos trombóticos en los extremos de la vida, relación que puede ser susceptible de un indagación mucho más minuciosa en futuros estudios; con todo esto, al codificar y sistematizar la variable tabaquismo (ex tabaquismo + Tabaquismo actual), no fue posible hallar diferencia con la proporción de muertes hospitalarias en pacientes con eventos trombóticos susceptibles de trombólisis. Es muy importante anotar que el antecedente de hipertensión arterial en mayores de 65 años fue el más frecuente, con un valor estadísticamente significativo, que se ajusta a los estudios médicos que se han hecho al respecto en tanto resultados y efectos del antecedente de hipertensión arterial. En términos de la efectividad a la intervención del tratamiento trombolítico en ECV, el mayor impacto se vio reflejado en los pacientes con casos severos; por su parte, el TEP lo podemos entender como una patología con una exposición infrecuente a trombolíticos que, aun así, presenta un éxito notable al momento de la reperfusión. Con todo esto, podemos concluir que a pesar del subregistro local, estas primeras relaciones descriptivas, apuntan a que la terapia trombolítica, es efectiva en el centro de referencia, lo cual no excluye un estudio detallado sobre el perfil de la seguridad el tratamiento, que sería motivo de otro estudio particular. En los 90 casos atendidos en el centro de atención de tercer nivel de la ciudad de Manizales, los cuales cumplieron con los criterios de inclusión definidos para nuestro estudio, divididos en tres grupos, a saber: (ECV) Enfermedad Cerebrovascular Isquémica, (IAM) Infarto agudo de miocardio y (TEP) Trombo embolismo Pulmonar, en los cuales encontramos que la patología más intervenida con terapia trombolítica farmacológica es IAM con un (64%), de los cuales el (60.9%) son hombres con una media de edad de 61.8 años (IC 95% 58.1 – 64.0), así mismo, el (4.3%), mayores de 85 años. Este estudio, nos arrojó como conclusiones y resultados que la mayoría de pacientes presentaron una media de edad de 59.8 años. No se encontraron diferencias estadísticamente significativas con respecto al antecedente de hipertensión arterial ni tabaquismo. Así mismo, el 35.9% de los casos de pacientes estudiados, sufrieron complicaciones y de estos, el (6.5%) sufrió sangrado mayor, con un sangrado intracraneal del (5.4%), de igual manera, no se reportó ninguna reacción alérgica, y la complicación más frecuente fue el sangrado en el sitio de punción con un (34.8%). El (87%) de los pacientes con TEP, lograron reperfusión, aunque solo el 68.8% recibió evaluación ecográfica pos-trombolisis. De igual manera, se registró un impacto significativo positivo en la progresión del (NIHSS) de ingreso frente a su medición a las 24 horas. NIHSS al ingreso 14.5 (IC 95% 12.2 – 16.7), a las 24 horas 8.7 (IC 95% 6.1 – 11.3). With all this, we can conclude that despite the local underreporting, these first descriptive relationships suggest that thrombolytic therapy is effective in the reference center, which does not exclude a detailed study on the safety profile of the treatment, which would be the subject of another particular study. In the 90 cases attended in the tertiary care center of the city of Manizales, which met the inclusion criteria defined for our study, divided into three groups, namely: (CVD) Ischemic Cerebrovascular Disease, (AMI) Acute myocardial infarction and (TEP) Pulmonary thromboembolism, in which we found that the pathology most intervened with pharmacological thrombolytic therapy is AMI with a (64%), of which (60.9%) are men with a mean age of 61.8 years (95% CI 58.1 – 64.0), likewise, (4.3%), older than 85 years. This study gave us as conclusions and results that the majority of patients had a mean age of 59.8 years. No statistically significant differences were found with respect to history of arterial hypertension or smoking. Likewise, 35.9% of the cases of patients studied, suffered complications and of these, (6.5%) suffered major bleeding, with an intracranial bleeding of (5.4%), likewise, no allergic reaction was reported, and the most frequent complication was bleeding at the puncture site with 1 (34.8%). (87%) of the patients with PTE achieved reperfusion, although only 68.8% received post-thrombolysis ultrasound evaluation.
