dc.description.abstract | spa: La supervivencia de pacientes con melanoma cutáneo (MC) invasivo ha sido muy poco estudiada en Colombia, y es insumo fundamental para la toma de decisiones en escenarios clínicos y de salud pública. Objetivo: Establecer los factores predictores de supervivencia de los pacientes diagnosticados con MC invasivo entre los años 2006 y 2015 en la ciudad de Manizales, según las características del paciente y del cáncer. Métodos: Estudio de cohorte de base poblacional que incluyó todos los casos de MC invasivo captados por el Registro Poblacional de Cáncer de Manizales (RPCa-M) entre los años 2006 y 2015 (n=132). Se realizó seguimiento a los pacientes hasta el 31 de diciembre de 2020; se calculó y graficó la supervivencia específica para MC invasivo con el método Kaplan-Meier para las variables sexo, edad, régimen de aseguramiento, topografía, subtipo histológico, grosor de Breslow, ulceración de la lesión y estadio clínico. Además, se ejecutó el log-rank test para determinar la presencia de diferencias estadísticamente significativas en supervivencia entre las diferentes categorías de las variables estudiadas, y una regresión multivariada de Cox con el fin de obtener factores predictores de supervivencia con sus respectivos hazard ratios (HR). Los cálculos estadísticos se efectuaron con el software Stata IC 14.2. Resultados: El 59,1% de los casos se presentaron en el sexo femenino. El subtipo histológico más frecuente fue el melanoma nodular (MN) seguido del lentiginoso acral (MLA) (25,8% y 18,2%, respectivamente). La mayoría de las lesiones se localizaron en miembro inferior y cadera. Se detectó una alta proporción de casos en estadio IV (19,7%), con grandes diferencias según sexo (29,6% H Vs 12,8% M). La supervivencia específica a 5 años de MC en Manizales es de 68,7% (IC 95% 59,9-76,0), con diferencias estadísticamente significativas en el análisis multivariado según edad (> 70 años HR 2,94 IC95% 1,22-7,06), subtipo histológico (SAI HR 24,62 IC95% 2,91-208,03; MN HR 20,93 IC95% 2,50-175,32; MLA HR 10,43 IC95% 1,22-88,46) y estadio clínico (EC-IV HR 17,13 IC 95% 7,10-41,35). Si bien en el análisis univariado también se encontraron diferencias estadísticamente significativas en supervivencia según grosor de Breslow, estado de ulceración y sexo, estas no se conservaron en el análisis multivariado. Conclusiones: En Manizales, aproximadamente 3 de cada 10 pacientes con MC invasivo fallecen en el primeros cinco años del diagnóstico, siendo mayor el riesgo de muerte para mayores de 70 años, subtipos histológicos SAI, MN, MLA y estadios clínicos avanzados (IV). Se debe enfatizar en las estrategias preventivas centradas en la comunidad y el talento humano en salud para promover la fotoprotección, y detectar y manejar oportunamente lesiones sospechosas en piel. | spa |
dc.description.abstract | eng: The survival of patients with invasive cutaneous melanoma (CM) has been very little studied in Colombia, and it is a fundamental input for decision-making in clinical and public health settings. Objective: To establish the survival predictors of patients diagnosed with invasive CM between 2006 and 2015 in the city of Manizales, according to the characteristics of the patient and the cancer. Methods: Population-based cohort study that included all cases of invasive CM captured by the Manizales Population Cancer Registry (RPCa-M) between 2006 and 2015 (n = 132). The patients were followed up until December 31, 2020; The specific survival for invasive CM was calculated and plotted with the Kaplan-Meier method for the variables sex, age, insurance regime, topography, histological subtype, Breslow thickness, ulceration of the lesion and clinical stage. In addition, the log-rank test was performed to determine the presence of statistically significant differences in survival between the different categories of the variables studied, and a multivariate Cox regression in order to obtain predictive factors of survival with their respective hazard ratios (HR ). Statistical calculations were performed with Stata IC 14.2 software. Results: 59.1% of the cases were female. The most frequent histological subtype was nodular melanoma (MN) followed by acral lentiginous (ALM) (25.8% and 18.2%, respectively). Most of the injuries were located in the lower limb and hip. A high proportion of stage IV cases was detected (19.7%), with large differences according to sex (29.6% H Vs 12.8% M). The 5-year specific survival of CM in Manizales is 68.7% (95% CI 59.9-76.0), with statistically significant differences in the multivariate analysis according to age (> 70 years HR 2.94 95% CI 1 , 22-7.06), histological subtype (SAI HR 24.62 95% CI 2.91-208.03; MN HR 20.93 95% CI 2.50-175.32; MLA HR 10.43 95% CI 1, 22-88.46) and clinical stage (EC-IV HR 17.13 95% CI 7.10-41.35). Although the univariate analysis also found statistically significant differences in survival according to Breslow thickness, ulceration status, and sex, these were not preserved in the multivariate analysis. Conclusions: In Manizales, approximately 3 out of 10 patients with invasive CM die in the first five years after diagnosis, with the risk of death being greater for those over 70 years of age, histological subtypes NOS, MN, MLA and advanced clinical stages (IV). Emphasis should be placed on preventive strategies focused on the community and human talent in health to promote photoprotection, and promptly detect and manage suspicious skin lesions. | eng |
