Descripción de características clínicas e histopatológicas renales de pacientes diabéticos con proteinuria en rango nefrótico

eng: The present study describes the demographic, clinical and histopathological characteristics in a population of diabetic patients with 24-hour proteinuria in the nephrotic range; finding that the demographic characteristics were similar to other studies in other latitudes. Different types of lesions were also identified in the renal biopsy that were grouped into 3 groups according to the findings (diabetic nephropathy only, non-diabetic nephropathy only or a combination of both), finding that the most frequent was group 3, followed by group 2, contrary to what was considered years ago, in which it has been considered that proteinuria of any degree in a diabetic patient is only a consequence of diabetic nephropathy. Additionally, when making exploratory comparisons between these groups, we found that the value of proteinuria in 24 hours was much higher in the group that presented only diabetic nephropathy; however, this difference was not statistically significant, possibly due to the low number of patients in the study. present study. One of the variables in which we found statistically significant differences was in the time of diagnosis at the time of renal biopsy, being higher in the group with isolated diabetic nephropathy, similar to that found in the literature, this indicates that in patients with proteinuria in nephrotic range with a long time of evolution of diabetes could be more likely to find diabetic nephropathy as a finding in the renal biopsy. This is a study in which we intended to show and call attention to medical personnel who care for diabetic patients, that not all kidney alterations that these patients present will be secondary only to diabetic nephropathy, and that attention should be paid to the atypical manifestations and indications for renal biopsy that allow an early diagnosis that may impact on the treatment and prognosis of patients. We consider that it is necessary to carry out similar investigations in our environment, extending the time of collection of patients, which allows obtaining a larger sample, since it would allow better characterization, make associations and even carry out predictive models similar to those proposed in previous studies, which are adjusted to the characteristics of the Colombian population. spa:El presente estudio describió las características demográficas, clínicas e
histopatológicas en una población de pacientes diabéticos con proteinuria de 24 horas en rango nefrótico; encontrando que las características demográficas fueron similares a otros estudios en otras latitudes. También se identificaron diferentes tipos de lesiones en la biopsia renal que se agruparon en 3 grupos de acuerdo a los hallazgos (nefropatía diabética únicamente, nefropatía no diabética únicamente o combinación de ambas), encontrando que el más frecuente fue el grupo 3, seguido del grupo 2, contrario a lo que se consideraba años atrás, en los que se ha considerado que la proteinuria de cualquier grado en un paciente diabético únicamente es consecuencia de la nefropatía diabética.
Adicionalmente al realizar comparaciones de manera exploratoria, entre dichos
grupos, encontramos que el valor de proteinuria en 24 horas fue mucho mayor en el grupo que presentaba nefropatía diabética únicamente, sin embargo dicha diferencia no fue estadísticamente significativa, posiblemente por el bajo número de pacientes del presente estudio. Una de las variables en la que encontramos diferencias estadísticamente significativas fue en el tiempo de diagnóstico al momento de la biopsia renal, siendo mayor en el grupo de nefropatía diabética aislada, similar a lo hallado en la literatura, esto indica que en pacientes con proteinuria en rango nefrótico con largo tiempo de evolución de la diabetes podría ser más probable encontrar nefropatía diabética como hallazgo en la biopsia renal.
Este es un estudio en el que pretendíamos mostrar y hacer un llamado de atención al personal médico que atiende pacientes diabéticos, que no todas las alteraciones renales que presenten dichos pacientes, serán secundarias únicamente a la nefropatía diabética, y que se debe estar atento a las manifestaciones atípicas y a las indicaciones de biopsia renal que permitan un diagnóstico temprano que pueda impactar sobre el tratamiento y pronóstico de los pacientes.
Consideramos que es necesario realizar investigaciones similares en nuestro
medio, ampliando el tiempo de recolección de pacientes, que permitan obtener
una muestra mayor, ya que permitiría mejor caracterización, realizar asociaciones e incluso realizar modelos predictivos similares a los propuestos en estudios previos, que se ajusten a las características de la población colombiana.
