dc.description.abstractspa:Objetivo: analizar la influencia del entorno construido sobre el temor a caer, mediado por la capacidad intrínseca, en personas mayores participantes en el estudio IMÍAS. Metodología: se realizó análisis secundario de datos derivado del estudio “The International Mobility In Aging Study (IMIAS)”. La muestra estuvo constituida por 2002 personas mayores. La variable dependiente fue el temor a caer medido a través de la escala FES-I; como variable independiente se tomó el entorno construido valorado a través de las características de la vivienda y los elementos construidos para la movilidad y el transporte en el barrio; las variables mediadoras fueron los cinco dominios de la capacidad intrínseca. Resultados: el temor a caer se incrementó a cuatro años en sus niveles moderado y alto en un 35,9% y 25,4% respectivamente (p=<0,0001). Las condiciones del entorno que predicen la presencia de temor a caer son: problemas raciales, étnicos o religiosos dentro del barrio (OR=3,1; IC95%:1,163 - 8,279); iluminación inadecuada en el barrio (OR=2,4; IC95%:1,406 - 4,277); problemas de entrada de agua en la vivienda (OR=1,8; IC95%:1,243 - 2,607); problemas de delincuencia dentro del barrio (OR=1,7; IC95%:1,050 - 3,063); no contar con parques de fácil acceso (OR=1,6; IC95%:1,117 - 2,451); falta de espacio en la vivienda (OR=1,5; IC95:1,134 - 2,046) y presencia aceras rotas dentro del barrio (OR=1,5; IC95%:1,148 - 1,796). La limitación de la movilidad, los síntomas depresivos y la fuerza de agarre débil producen efecto de mediación completa y parcial entre el entorno construido y el temor a caer. Conclusión: los predictores encontrados se relacionan con problemas en la estructura física de la vivienda, difícil acceso al barrio y problemas sociales. Tres dominios de la capacidad intrínseca son mediadores que predicen la variación parcial o completa entre el entorno construido y el temor a caer.spa
dc.description.abstracteng:Objective: to analyze the influence of the built environment on fear of falling, mediated by intrinsic capacity, in elderly people participating in the IMIAS study. The dependent variable was the fear of falling measured through the FES-I scale; the independent variable was the built environment assessed through the characteristics of the home and the elements built for mobility and transportation in the neighborhood; the mediating variables were the five domains of intrinsic capacity. Results: fear of falling increased at four years in its moderate and high levels by 35.9% and 25.4% respectively (p=<0.0001). The environmental conditions that predict the presence of fear of falling are: racial, ethnic or religious problems within the neighborhood (OR=3.1; 95%CI:1.163 - 8.279); inadequate lighting in the neighborhood (OR=2.4; 95%CI:1.406 - 4.277); water entry problems in the home (OR=1.8; 95%CI: 1.243 - 2.607); crime problems within the neighborhood (OR=1.7; 95%CI:1.050 - 3.063); lack of easily accessible parks (OR=1.6; 95%CI:1.117 - 2.451); lack of space in the dwelling (OR=1.5; 95%CI:1.134 - 2.046) and presence of broken sidewalks within the neighborhood (OR=1.5; 95%CI:1.148 - 1.796). Mobility limitation, depressive symptoms and weak grip strength produce complete and partial mediation effect between the built environment and fear of falling. Conclusion: the predictors found are related to problems in the physical structure of the dwelling, difficult access to the neighborhood and social problems. Three domains of intrinsic capacity are mediators that predict partial or complete variance between the built environment and fear of falling.eng
