dc.description.abstractspa:A pesar de los beneficios de los fármacos, algunos son considerados medicamentos potencialmente inapropiados (MPI) en los ancianos dado el incremento del riesgo de eventos adversos. Los adultos mayores son especialmente vulnerables al daño causado por medicamentos. La evidencia actual sobre los efectos de los MPI en los adultos mayores sigue siendo controversial. El propósito de este estudio fue determinar el impacto de los MPI en la salud de los ancianos de una cohorte internacional. Se trató de un estudio observacional, multicéntrico, retrospectivo, longitudinal de 4 años de seguimiento (2012-2016), de adultos mayores de la comunidad. Se caracterizó la salud de los ancianos mediante 10 indicadores agrupados en 5 categorías (salud física, bienestar psicológico, función cognitiva, participación social y función física). Se siguieron 820 participantes en 5 ciudades, la edad promedio fue 69,07 años; 50,9% fueron mujeres. La proporción de participantes con SPPB bajo en el grupo de exposición fue de 24,24% en 2012 y 35,15% en 2016, lo cual corresponde con un aumento del 65% en la proporción de los participantes con SPPB bajo (Valor p = 0,000; OR: 12,502, IC 95% 6,827-22,896). En el modelo multivariado ajustado con GEE, las probabilidades de tener una función física baja fueron 2,084 veces mayores en el grupo de exposición vs los no expuestos después de ajustar por potenciales confusores (RRajus 2,084; IC 95% 1,589-2,732). Concluimos que la exposición sostenida a MPI está relacionada con una funcionalidad física baja en el largo plazo, y que esta relación no es explicada por otras variables o factores de confusión demográficos y clínicos.spa
dc.description.abstracteng:Despite the benefits of drugs, some are considered potentially inappropriate medications (PIM) in the elderly due to the increased risk of adverse events. Older adults are especially vulnerable to harm caused by medications. The current evidence on the effects of PIM in older adults remains controversial. The purpose of this study was to determine the impact of PIM on the health of elderly individuals in an international cohort. It was an observational, multicenter, retrospective, longitudinal study of a 4-year follow-up (2012-2016) of community-dwelling older adults. Elderly health was characterized by 10 indicators grouped into 5 categories (physical health, psychological well-being, cognitive function, social participation, and physical function). A total of 820 participants were followed in 5 cities, with an average age of 69.07 years, and 50.9% were women. The proportion of participants with low SPPB in the exposure group was 24.24% in 2012 and 35.15% in 2016, corresponding to a 65% increase in the proportion of participants with low SPPB (p-value = 0.000; OR: 12.502, 95% CI 6.827-22.896). In the adjusted multivariate model with GEE, the odds of having low physical function were 2.084 times higher in the exposure group compared to the non-exposed group after adjusting for potential confounders (adjusted RR 2.084; 95% CI 1.589-2.732). We conclude that sustained exposure to PIM is associated with long-term low physical function, and that this relationship is not explained by other demographic and clinical variables or confounding factors.eng
